Intake Evaluation with Brief Diagnostic Test Battery-$375 (80 min.)
Basic Intake Evaluation (Does Not Include Tests)-$275 (80 min.)
Group Therapy Intake -$210 (50 min.-Includes some assessment measures)
Individual/Family Psychotherapy -$170 (50 min.)
Group Psychotherapy -$70 (65 min.)
Case Management (E-mail, Phone Calls, Treatment Coordination)-$50/per 15 minute increment
Legal Testimony/Preparation of Legal Documents-$425/hour
Missed Appointment Charge (Does NOT Apply to Group Therapy)-Full Service Fee
Attendance at School Meeting/Conference-$190/hr. (Minimum of 1 hour billed-Includes Metro Louisville/Southern IN)
Phone Consult/Therapy-$85/Half-Hour
Psychological Testing-$325 per testing unit
Full Psychological Assessment Battery-$3,000
Abbreviated Psychological Assessment Battery-$2500
Assessment Batteries can be customized to the clinical needs of the individual client. Quotes for specific assessments are offered at no cost.
